Insurance Plans
Ogden Clinic participates with most area insurance plans, and we will be happy to submit your insurance claims for you. Any questions regarding your specific coverage, including referrals and authorizations, should be directed to your insurance company or your company benefits administrator. You are responsible for any amount not covered by your plan. Below is a partial list of insurances we carry. If your insurance is not listed, you can make a request through our Patient Comments:
